Charitable Solicitations

 
Charitable Solicitations filings must be received no later than May 31.

You will need just 3 items to complete the Charitable Solicitations form: the postcard & instructions from WSPTA, your 990 or 990EZ tax form, and the Charitable Solicitations form (downloadable online.)

Hiring Independent Contractors

Planning to hire an instructor to teach a course?  PTSA needs to have a contract in place, a
W-9 from the instructor (unless they are incorporated) and a final invoice from the instructor so that we can issue payment.  Use these steps and forms for a seamless process!  

  1. Create a contract with the instructor detailing things like the subject of the class, the approximate size, and the date.  Contract
  2. Ask the instructor to print out and fill out an IRS form W-9.  The PTSA needs to report all payments over $600.00 to the IRS for independent contractors that are unincorporated.  Form W-9
  3. Submit the signed contract and W-9 to the treasurer for filing and for use in preparing the end-of-year taxes. When checks come in for people taking the class, fill out a deposit register, listing who the checks were from, and the amounts.  Deposit Register
  4. At the end of the class, have the instructor fill out an invoice and make sure it gets to the treasurer.  The treasurer will the make sure the payment gets to the instructor.  Invoice
